Clearly defined learning objectives, both short and long term.
Frequent, brief, and focused assessments that allow for immediate feedback.
Consistent form and grading/scoring (no grades taken), so that students know what to expect and how to determine if they have mastered the concepts.
Explanation of concepts being assessed, so that students know which concepts they have mastered and which concepts they have not.
Intervention and enrichment opportunities after each formative before they move on to the next concept and get further behind.

Who: Teachers, administrators
What: Decide on a form, frequency, feedback, and intervention & enrichment situation to make the formative assessments effective. Formative assessments are effective when they help students and teachers assess student learning and adjust future lessons according to the students' understanding.
When: Can be started at any time, but best implemented at the beginning of the year or the beginning of a new unit.
Where: Can be used in any classroom at any age, because the form, feedback, and intervention & enrichment situation can be constructed depending on maturity level.

(i.e. rather than deciding that the entire district assessment plan needs to be rewritten, the change may be the introduction of formative assessment into instructional planning.)
